Administrative Case #2024-I-031, Effective 01/30/2025

Mr. LeBert engaged in unprofessional conduct engaging in a romantic relationship with a patient.  Due to the serious nature of the allegations in the complaint, the Investigative Committee determined that Respondent should not practice physical therapy during the pendency of the investigation and/or administrative proceedings before the Board. Mr. LeBert, through counsel, agreed to a suspension of his license pending further proceedings before the Board.

The suspension is effective as of December 27, 2024.

 

Kelly Campbell, PT, License # 09533R, Shreveport, LA

Administrative Case# 2024-I-024, Effective 04/03/2025

Ms. Campbell's license was summarily suspended pending an Administrative Hearing scheduled for May 7, 2025. Emergency action was taken due to the recommendations set forth in a fitness for duty evaluation completed by Ms. Campbell, in which the treatment provider determined she was not fit to practice at this time until further treatment was completed.

 

Kelly Campbell, PT, License # 09533R, Shreveport, LA

Administrative Case# 2024-I-024, Effective 05/08/2025

Ms. Campbell's license is suspended until such time that she completes a board-approved inpatient treatment program for chemically dependent professionals and follows the recommendations of the treatment team.  She is not fit to return to the practice of physical therapy until she has completed treatment, has a continuing care plan in place, has signed a program agreement, and has been determined fit to return to duty.

 

Gabriel Sonnier, PT, License # 11763, Thibodaux, LA

Administrative Case# 2025-I-014, Effective 05/09/2025

Mr. Sonnier’s license was summarily suspended pending an Administrative Hearing.  Emergency action is required due to the nature of the allegations against Mr. Sonnier. On March 19, 2025, the Board received notification that Mr. Sonnier had been arrested on March 16, 2025, and charged with two counts of felony indecent behavior with juveniles, in violation of Louisiana Revised Statute 14:81, and three counts of felony obscenity, in violation of Louisiana Revised Statute 14:106.

 

Gabriel Sonnier, PT, License # 11763, Thibodaux, LA

Administrative Case# 2025-I-014, Effective 06/18/2025

Mr. Sonnier is suspended until such time that he complies with all recommendations of the evaluation team as stated in his fitness for duty evaluation report dated May 1, 2025.   Upon completion of in-patient treatment, Mr. Sonnier shall submit to a forensic psychosexual evaluation to determine his fitness for duty and he agrees to follow all treatment recommendations resulting from the treatment program and forensic evaluation.  Upon reinstatement after the suspension period, his license shall be on probation for a minimum of five years.  He shall participate in the RPTP program.  During the probationary period he shall comply with monitoring visits and shall submit to supervised clinical practice.

 

Christian Koestler, PTA, License # A11898, Shreveport, LA

Administrative Case# 2024-I-022/2025-I-017, Effective 05/28/2025

Mr. Koestler’s physical therapist assistant license was summarily suspended pending an administrative hearing. Emergency action is required due to the recommendations set forth in a fitness for duty evaluation completed by Mr. Koestler, in which the treatment provider determined he was not fit to practice at this time until further treatment was completed.

 

Steven Powell, PTA, License # A8990, Broussard, LA 

Administrative Case # 2024-I-032, Effective 05/08/2025

Mr. Powell fraudulently documented the performance of and signed on behalf of patients for treatment visits which purportedly took place via home health provider.  Respondent created vital signs and other entries for treatment documentation, knowingly forged signature of the patient, and submitted documentation for a treatment that never happened.  Mr. Powell's license is suspended for six months, followed by a three-year probation.  Within the first six months of his probationary period, Mr. Powell must complete Professionalism and Ethics in Physical Therapy course via APTA learning center.  During probationary period, he shall not work in the home health setting.  During the probationary period, he shall have on-site supervision by the supervising physical therapist to include daily face-to-face communication and on-premises observation of patient care.
