Military Licensing Information

Louisiana only requires that the state jurisprudence exam be completed both prior to a Compact privilege purchase and at the time of renewal. Compact holders are not responsible for any further CE requirements.

To move forward with creating a Compact Privilege User account to take the Jurisprudence exam and report your contact information, create an account. 

If you are currently licensed as a PT or PTA in good standing in another state, district, or territory of the U.S. and are requesting a license because you or your spouse has military orders to relocate to Louisiana, you must submit the following:

1. An online application. Once the online application has been received, the Licensing Analyst will update your checklist within 2 business days to remove items which are not necessary for the SCRA process. Create your account;
2. Background check. Please be aware that background check results cannot be received by the Board office prior to the submission of an online application;
3. A copy of the member's military orders showing relocation to Louisiana;
4. If the applicant is a military spouse, a copy of the military spouse's marriage license; and
5. The notarized affidavit affirming under penalty of perjury that:
   1. you are the person described and identified in the application;
   2. all statements in the application are true, correct, and complete;
   3. you understand the scope of practice for a physical therapist or physical therapist assistant licensed in this state and will not perform outside of that scope of practice; and
   4. you are in good standing in each state in which you hold or have held a PT and/or PTA license.
   5. you understand that “good standing” means that your license(s) has not been revoked or had discipline imposed by any State, does not have an active investigation(s) relating to unprofessional conduct pending in any State relating to it, and has not been voluntarily surrendered while under investigation for unprofessional conduct in any State.
